On the podcast this week, John discusses the concept of Direct Primary Care (DPC). First appearing in the early 2000’s, this type of practice is characterized by the elimination of third party payers. In most cases, insurance billing is replaced by a subscription or “membership” model in which patients prepay for their care on a monthly or quarterly basis.
The typical DPC practice agrees to provide a comprehensive set of outpatient services that meet the needs of most patients. Hospitalizations and high-cost surgeries are generally covered by traditional insurance.
Under this model, the elimination of insurance billing results in lower overhead, less paperwork, fewer employees, and better access by expanding the use of texting, phone calls, email, and telemedicine. Office visits are generally longer and more comprehensive with less time spent on paperwork. While originally designed for primary care, there are examples of specialty-based DPC-style payment arrangements.
John describes why this can be the solution to burnout and a way to recover the joy of practicing medicine while earning a very good income. He also explains that creating a DPC practice requires extensive planning and significant financial resources, but less than opening a traditional practice. And he offers his thoughts on transitioning from employment to your own DPC practice.

Robert J. Cooper, MD, is an attending physician in the Department of Endocrinology at Holyoke Medical Center in Holyoke, Massachusetts. He received his medical degree from Albert Einstein College of Medicine, Bronx, New York, and completed residency training in internal medicine at Long Island Jewish Hospital, New Hyde Park, New York. He completed fellowship training in endocrinology at Long Island Jewish Hospital and Winthrop–University Hospital, Mineola, New York.
He lectured this past year at SEAK on combating physician burnout with nonclinical diversification, including knowledge consulting. He provides keynote lectures and/or provide workshops to physician groups interested in combining nonclinical and clinical activities. Dr. Sanj Katyal's journey began with a solid foundation in science and medicine. He earned a Bachelor of Science in Chemical and Biomedical Engineering from Carnegie Mellon University and a Doctor of Medicine (MD) from New York University School of Medicine. While his early career in radiology and clinical operations brought professional success, it also exposed him to the limitations of traditional Western medicine in addressing the deeper causes of human discontent.
The turning point came when Dr. Katyal discovered Rapid Transformational Therapy (RTT) and became a certified hypnotherapist and RTT therapist in 2024. His transformation through RTT, including overcoming a deep-seated fear, reinforced his belief in the power of addressing subconscious limiting beliefs.
During today’s interview, Dr. Katyal describes the limitations of positive psychology and the benefits of rapid transformational therapy. He then goes on to describe the explosion in mental illness in children and young adults since 2010.

Dr. Helen Rhodes is an obstetrician/gynecologist who was first interviewed in 2019. She became dissatisfied with her practice, so she completely redesigned her clinical career, and added a nonclinical component.
Helen received her medical degree from the Texas Tech University Health Sciences Center and completed the Obstetrics and Gynecology Residency Program at the University of Texas Health Science Center in Houston. For her first job, she landed a position in a traditional academic setting. She later tried private practice.
Finally, she left traditional practice and began exploring several alternative practice options, while also developing a part-time job as medical director for a plasmapheresis company. During our interview, she describes how she found each position, and how she created her novel career by combining several different clinical and nonclinical pursuits.

In this week’s podcast episode, John addresses this question: Even if I’m burned out and frustrated, are there ways to stay in clinical medicine while achieving balance, satisfaction, and enjoyment? During this presentation, he discusses the following 5 options for remaining in clinical practice.
Take another stab at addressing the circumstances leading to dissatisfaction by aggressively renegotiating your employment contract. Review your contract and identify the major issues causing your frustration. Develop a strategy for renegotiating those issues with your attorney. These typically involve vacation time and covering for others’ vacations, work hours, salary, restrictive covenants, and inadequate pay for collaborating with midlevel providers.
If you’re looking for a new job, consider engaging an agent to represent you in contract negotiations with a new employer, often at no cost to you.
Start a new practice or join one that does not bill insurance companies. Direct Primary Care and Concierge practices, and narrow-focused practices often enable the physicians to work on cash-only, fee-for-service, or membership models, and to create a better lifestyle and income.
Almost any of us could start or transition to a narrow focused practice. John’s colleague was a full-time general pediatrician who transitioned to caring solely for patients with attention deficit disorder. He uses standard protocols for evaluation and management, and is an expert at billing for these services. He reduced his staff and overhead and his satisfaction and income have improved.
Finally, for those who want to achieve many of the above advantages, and others, without starting a practice, working as a micro-corporation may be a great choice. Incorporating and working as an independent contractor for a large group or hospital system provides more tax benefits, flexible investment opportunities, and off-loading the expenses for rent and staffing.
