    Dr. Yoni Rosenblatt sits down with Dr. Jamie Quinto, a physical therapist in the Bethesda clinic who has become one of the most requested therapists in the practice about a year into her career.

    Dr. Roger St. Onge: Reading Patterns, Neurovital Signs & Why Extremity Pain Comes From the Spine

    Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Roger St. Onge, a residency and fellowship trained physical therapist who now coaches clinicians on how to think through an evaluation rather than how to collect more tools.

     

    The conversation opens on patterns. Roger makes the case that most of the diagnostic picture comes from the subjective, that the objective exam needs at least two differentials to eliminate, and that testing should be built to prove you wrong instead of confirming what you already believe. He walks through the MAPs he listens for: the movements, activities, positions, and situations that fill a patient's day.

     

    From there he treats the nervous system as a set of vital signs rather than a screen. Reflexes, myotomal strength, and neural tension sensitivity show what symptoms cannot, and Roger explains how he grades neurological fatigability, why he reads pain on a 24-hour clock, and how an athlete a year past ACL surgery finally passed return to sport testing once his lumbar spine was addressed.

    Dr. Gregory DiFelice: ACL Repair vs Reconstruction, the Preservation First Algorithm & the BEAR Implant

    Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Gregory DiFelice, an orthopedic surgeon at Hospital for Special Surgery who has spent close to 20 years repairing ACLs rather than replacing them.

     

    He lays out Preservation First, his algorithm for sorting ACL tears by tear type and tissue quality, and the four lanes it runs through: repair, BEAR, augmentation, and reconstruction as a last resort. He also explains why the field abandoned repair decades ago, what the 1993 paper that buried it actually said, and why he thinks the current direction of ACL surgery is big, bigger, and biggest.
