- 1 hour 49 secondsYou Know His Research. But Do You Know Paul Hodges?
You might not immediately recognize the name Paul Hodges. But we can almost guarantee you've been taught his work.
And if you're attending PelviCon (either in-person or with our Live Virtual option) you'll get the chance to learn from him directly!
If you've learned that...š The pelvic floor and diaphragm work together
š The pelvic floor is part of a larger pressure-management system
š Timing and coordination matterānot just strength
š The abdominals and pelvic floor don't function independently
š Intra-abdominal pressure isn't inherently bad
š Pain is far more complicated than simply "tissue vs. brain"
...then you've been influenced by research Professor Paul Hodges and his collaborators have spent decades developing.
And that's why we were SO excited to sit down with him.Ā
Paul is one of the most influential physiotherapy researchers in the world, and in this episode we get to nerd out with him about the science that has fundamentally shaped how we practice.
We talk about:
š¬ What we got wrong about his early motor-control research ā and how complex science gets oversimplified when it reaches the clinic.
šØ Why pressure isn't the enemy. Paul introduces the Swedish concept of lagom: we need the appropriate amount of pressure for the taskānot simply less pressure.
āļø Why we can't just extrapolate female pelvic floor rehab to men. Paul's team has been investigating what actually creates continence after prostatectomy and how that should change rehabilitation.
š§ Why we've oversimplified chronic pain. We dig into nociceptive, neuropathic and nociplastic paināand why chronic pain doesn't automatically mean the tissues are irrelevant.
𤯠The problems with ācentral sensitization.ā Including Nicole getting a little surprise when Paul explains the limitations of the Central Sensitization Inventory!
š How research actually makes its way into clinical practiceāand why Paul believes research should ultimately change outcomes for real people.
And maybe our favorite theme of the entire conversation:
Complexity doesn't mean we throw our hands up.
Our job is to understand that complexity well enough to find the clinically meaningful pieces that help the person sitting in front of us.
šļø Learn From Paul LIVE at PelviCon 2026!
If this conversation gets your clinical brain firing, just wait until PelviCon. Ā (You can still get your tickets at www.pelvicon.com/virtual)

and
Same Symptoms, Different Pain: Phenotyping Pelvic Pain in Clinical Practice

About Professor Paul Hodges
Professor Paul Hodges is a physiotherapist, neuroscientist and professor at the University of Queensland whose research has profoundly influenced our understanding of motor control, biomechanics, pain, continence, breathing and pelvic floor function.
His work spans decades and hundreds of scientific publications, but what makes it so relevant to us is how many of the principles we now take for granted in pelvic rehab have roots in questions Paul and his collaborators were asking long before those concepts became commonplace.Ā

When does a cash-based pelvic health practice actually need a fancy CRM?
For most of us? Never.
And yesāwe've made this mistake ourselves. š¬
In this episode, we're breaking down what a CRM actually is, how it's different from your EMR and email platform, and why adding more technology often creates more problems instead of solving them.Ā
We get into:
š Why you probably already have a CRM (even if it's just a Google Sheet!)
š The difference between your CRM, EMR and email platform
š Why PelvicSanity went from a simple Call Log ā fancy CRM ā right back to the Call Log
š The hidden costs of tech stacking and automation
š Why you shouldn't build your business systems around a specific admin
š The question you should ask before adding ANY new software, system or complexity
The bigger principle?
Fight for simplicity.
Before adding something new to your business, get crystal clear:Ā What problem am I actually trying to solve?
Then ask whether this is really the simplest, best way to solve it.
Because in more than 1,000 pelvic health business owners we've worked with, we've never once thought:
"You know what's really holding this business back? They need a better CRM." šĀ

How can we help save the lives of our pregnant and post-partum patients?Ā What about doing more than just resolving their reported symptoms?
We can't wait for you to hear this conversation with Katherine Sylvester - she has spent her entire career challenging the status quo and pushing for better results for her patients. Ā
After experiencing postpartum preeclampsia herself, Katherine began asking how wearable data could help women recognize problems earlier, advocate for themselves and make better decisions during pregnancy and postpartum. That eventually grew into Operation M.I.S.T. and her work helping women use their own data to better understand their bodies.Ā
And that's just one part of this conversation!
We talk about:
š How Katherine's own postpartum experience led to Operation M.I.S.T.
š Using wearable data to identify concerning changes before they become major problems
š Teaching women to self-monitor and make data-driven decisions
š Why postpartum return to exercise needs to be individualized
š Her mission to reduce unnecessary C-sections
š Why we're barely scratching the surface of what pelvic PT/OT can do with scar management
š What happens when you stop complaining about problems and start asking, āWhat can I do about this?ā
Katherine is bringing two completely new topics to the PelviCon stage this year: using data to address maternal mortality and better understanding and treating scars in pelvic rehab. We can't wait for you to learn from her!Ā

What if the first clinician you hire isn't as good as you?
It's one of the biggest fears we hear from successful solopreneursāand it's exactly what Nicole struggled with when we started growing PelvicSanity.
Because hiring your first clinician isn't just adding another person to the schedule.
It's changing your entire role.
Suddenly, you have to trust someone else with your patients, your reputation, your culture and something you've worked incredibly hard to build.
And yes...Nicole struggled with this enough that she eventually went to therapy to work through some of the control issues that came with becoming a leader. š¬
In this episode, we're talking about the mindset shifts required to successfully go from solopreneur to multi-clinician owner, including:
š Why being busy isn't a good enough reason to hire
š "No one will ever be as good as me" (and what to believe instead)
š Why your next clinician can actually enhance your reputation
š Raising the floor with great systems, onboarding and mentorship
š Raising the ceiling by helping your clinicians become incredible
š Why your clinicians don't need to be perfect on Day 1
š The fear that you'll invest in someone...and they'll eventually leave
Ultimately, the traits that made you a successful solopreneurācontrol, high standards, personal responsibilityācan become the exact things that prevent you from building a great team.
Growing beyond yourself requires becoming a different kind of business owner.

If you've ever felt like you've "tried downtraining" and your patient still isn't getting better... this episode is for you.
We're also wrapping up our recent series on internal exams, the gold standard, and access to quality care before shifting back into one of our favorite clinical topics: downtraining.
The biggest mistake we see?
Thinking of downtraining as a single intervention.
In this episode, we introduce the updated framework from our completely re-recorded Downtraining Masterclass, including the three different "levers" of downtraining and how they help you identify why someone's pelvic floor is overactiveānot just that it is.
In this episode we discuss:
ā A quick recap of our recent series on internal exams & the gold standard
ā Why "I've tried downtraining" usually isn't true
ā The 3 Downtraining Levers:
- Ā Pelvic Floor DirectĀ
- Ā Pelvic Floor IndirectĀ
- Ā Nervous SystemĀ
ā How to choose the right intervention for the right patient
ā When to stay the course... and when it's time to pivot
If you treat patients with pelvic pain, overactivity, urinary urgency, constipation, or other pelvic floor dysfunction, this framework will immediately change how you think about treatment.

Let's cut through the noise....If someone you loved needed pelvic floor therapy tomorrow, where would you send them?
š¤ Are there actually different levels of pelvic health care?
We believe the answer is yes.
The interesting part? Most pelvic therapists already act as though these different levels existāwe've just never had a common language to describe them.
Think about it...
If your spouse, your parent, or your child developed pelvic floor dysfunction tomorrow, who would you recommend?
Most of us wouldn't say:
ā "Just find any ortho PT."
ā "Start with telehealth or try an online program."
ā "Here's a great Instagram account."
We'd recommend a pelvic floor specialist.Ā Someone who is trained and able to perform both an internal and external evaluation and hands-on treatment.
That instinct reveals something important: we already recognize that different models of care have different capabilities.
In this episode, we introduce what we're calling the Five Levels of Pelvic Health Careāa framework for thinking about the different therapeutic ceilings of pelvic health, from free educational resources all the way to the current gold standard of in-person pelvic floor specialists.
In this episode, we discuss:
ā Why different models of care have different capabilities
ā What we mean by a model's therapeutic ceiling
ā The Five Levels of Pelvic Health Care
ā Why every level has valueāand expands access in different ways
ā Why preserving the gold standard matters while thoughtfully increasing access
This isn't about ranking clinicians or criticizing different models of care.
Every level has passionate clinicians doing their absolute best for patients.
Every level has an important role to play.
But patients deserve honesty about what each model canāand cannotāprovide so they can make informed decisions about their care.
If you've been following this series, this episode brings everything together and introduces a framework we hope helps move the conversation around standards of care forward.
There's only two big requirements for us:
- Be honest about your therapeutic ceiling.
Don't present your level as though it replacesāor is equivalent or superior toāa higher level. - Be exceptional within the level you've chosen.Ā No matter what setting you've chosen to practice in, strive to learn, grow and provide incredible care.

- 1 hour 13 minutesWhat Do We Actually Gain From an Internal Pelvic Floor Evaluation?
To be honest, we never imagined we'd need to explain this.Ā
Fair warning: this episode gets a little nerdy.Over the last two episodes, we've talked about why we believe internal pelvic floor evaluation remains the gold standard in pelvic rehabilitation. This week, we're answering the obvious follow-up question:
What do we actually gain from performing one?
For those of us who were trained to perform internal evaluationsāand who use them every day in practiceāthe value seems almost self-evident. It's become such a fundamental part of how we think, reason, and treat that we rarely stop to ask why. It's also where our profession has focused its education, research, and clinical training for more than 40 years.
But recent conversations in our field have challenged us to step back and articulate something we've largely taken for granted.
So that's exactly what we do in this episode.
We break down the first principles of an internal pelvic floor evaluation:Ā
- How it reduces uncertainty
- Improves diagnostic accuracy
- Helps individualize treatment
- Identifies findings that might otherwise be missed
- Gives patients understanding and validation
This is why the ability and willingness to perform an internal evaluation - when clinically relevant and with informed consent - has remained the gold standard in pelvic rehabilitation for decades.
If you've ever struggled to explain why internal evaluation mattersāor simply want to think more deeply about one of the defining skills of our professionāthis episode is for you.
International Continence Society Paper
In the episode we mention a paper from the ICS that was extremely helpful in listing out many of the areas where we gain information, specificity or new information.Ā You can review the article in full here: https://mail.google.com/mail/u/3/?ui=2&ik=c052c6fa3f&attid=0.1&permmsgid=msg-a:r7722291455026827186&th=19f9addb07427973&view=att&zw&disp=inline&acrobatPromotionSource=gmail_chrome-list

"Access to care" sounds like something everyone should support.
So why do we think it's become one of the most misleading phrases in pelvic health?
In this episode, we argue that we've been solving the wrong problem.
Because the goal isn't access to care.Ā The goal is access to quality care.
We discuss:
- Ā Why lowering the standard of care isn't the same as increasing accessĀ
- Ā The difference between access and qualityĀ
- Ā Why patients are still falling through the cracks despite having "access"Ā
- Ā The six biggest opportunities to improve pelvic healthcareĀ
- Ā Why raising standardsānot lowering themāis how we truly help more peopleĀ
If our profession wants to serve more patients, the answer isn't diluting what makes pelvic health special.
It's building more clinicians, better businesses, stronger education, and higher standards.Ā

Is internal evaluation the gold standard in pelvic health?Ā We believe the answer is yes.
Lately, we've seen more clinicians claiming that internal pelvic floor evaluation isn't necessaryāor isn't the gold standard of pelvic rehab. We don't think this is just another social media debate.
We think it's one of the most important conversations our profession needs to have.
We're going to be diving deep into this topic.Ā But for Part 1, we discuss:
- Ā Why this issue matters so much for the future of pelvic healthĀ
- Ā What the pioneers of our profession fought to establishĀ
- Ā The strongest arguments against internal evaluationāand where we think they fall shortĀ
- Ā Why "access to care" shouldn't mean lowering the standard of careĀ
- Ā Why extraordinary claims require extraordinary evidenceĀ
(Also, the caveat we shouldn't need....Every internal evaluation should be clinically appropriate and performed only with informed patient consent.Ā Of course.)
Instead, this episode asks a bigger question:Ā What should the gold standard actually be for pelvic rehab?
